Rental Protection

LOSS DAMAGE WAIVER ADDENDUM TO RENTAL CONTRACT

You (lessee) shall, at your expense, provide and maintain protection against loss, damage, and/or destruction to the leased equipment, for its full value. Alternatively, you may elect to cover this responsibility for loss, damage and/or destruction to the leased equipment via a Loss Damage Waiver. To the extent that such loss, damage, or destruction results from your gross negligence or willful misconduct, any limitation of your responsibility provided by the Loss Damage Waiver or otherwise shall be void. Before deciding whether to accept the Loss Damage Waiver, you may wish to determine whether your own insurance affords you comparable coverage for damage to the leased equipment and the deductible amount you will be responsible for under your insurance.

The purchase of this Loss Damage Waiver is not mandatory and may be declined. The fee for this Loss Damage Waiver will be reflected on our invoice or other notice to you. Waivers hereunder do not constitute insurance.

Lessee is responsible for the first $5,000 of any valid claims in the form of a deductible.
Multiple deductibles may be applied separately in the event of multiple incidents of loss that occurs during the rental period.

CUSTOMER RESPONSIBILITIES

I am aware I am responsible for changes that may occur for rental overages, fuel, cleanup and damages if equipment is not returned in the same condition as it was upon leaving Owner’s location. I am also responsible for any deductibles or damages not covered by my insurance or pre-purchased LDW. I authorize Owner to collect payment for additional charges resulting from my rental using the credit card/debit card I have on file or in absence I will reimburse Owner for any charges within 10 days.

I have read and agree to the terms stated in the rental contract. I am also aware that I am responsible for any additional charges that may occur from rental usage overages, fuel, cleanup and damages if the equipment is not returned in the same condition as it was upon leaving Wolf Kubota’s location whether it is picked up or delivered. I am also responsible for any deductibles or damages not covered by my insurance or not covered by the loss damage waiver (LDW). All LDW insurance must be purchased prior to the rental. I authorize Wolf Kubota to collect payment for additional charges resulting from my equipment rental. I authorize Wolf Kubota to charge the credit/debit card on file to collect on any of the aforementioned additional charges. In absence of a credit card on file, I agree to reimburse Wolf Kubota for any of the additional charges within 24 hours of date of charge. I shall be liable for all reasonable and actual expenses including but not limited to attorney’s fees incurred by Wolf Kubota in the collection of monies.
